Round 1 Instruments:
These documents are the Round 1 instruments used to collect baseline data from approximately 12,000 individuals during 2000-2001. Patient data were collected via computer assisted telephone interviews (CATI) and mailed questionnaires to optimize response rates. View Instruments

Round 2 Instruments:
These documents are the Round 2 instruments used to collect follow-up data from approximately 9,000 individuals during 2002-2003. They hold the same general description as the baseline. Any variations and/or additional documents used are noted. Patient data were collected via computer assisted telephone interviews (CATI) and mailed questionnaires to optimize response rates. Round 3 Instruments:
These documents are the Round 3 instruments used to collect second follow-up data from approximately 6,000 individuals during 2005. They hold the same general description as Round 1 (baseline) and Round 2 (follow-up). Any variations and/or additional documents used are noted. Patient data were collected primarily via mailed questionnaires with one site using computer assisted telephone interviews (CATI) to optimize response rates. View Instruments

Cardiovascular Disease (CVD) Risk Factor Instruments:
This questionnaire was used to survey those at high risk for CVD events and a comparison group of well-controlled patients. These focused surveys are cross-sectional in design and focus on reasons for poor intermediate outcomes (i.e., poor levels of control of A1C, blood pressure, and LDL-C). Data were collected from 1,137 individuals during 2005-2006. Patient data were collected via computer assisted telephone interviews (CATI) and mailed questionnaires to optimize response rates.
