- Written Patient Survey (English version)
A modified version of the Round 2 survey described above to evaluate the quality of care that people with diabetes receive, changes in treatment or care since the previous survey, self-reported pain and depression, and questions to determine hospitalization for key events such as stroke and MI. This survey only contains 66 questions including skip patterns, and takes approximately 40 minutes to complete.

- Short Written Patient Survey (English version)
A brief questionnaire (a total of 7 questions) to help identify health events such as heart attacks and strokes in persons with diabetes. Used as a last attempt at data collection.
