Round 2 Instruments

  • Written Patient Survey (English version)
    Written survey used at baseline with additional questions to assess health-related quality of life, satisfaction with care, barriers to care, and socioeconomic status as well as questions to determine endpoints of interest: myocardial infarction, stroke, and renal failure.  Contains a total of 103 questions including skip patterns and takes approximately 50 minutes to complete.

    • Spanish Version
      Please note, footnote references are for questions in English that our study researchers had translated into Spanish.

  • Short Written Patient Survey (English version)
    An abbreviated version of the written patient survey described above.  A total of 50 questions including skip patterns used as a last attempt at data collection; takes approximately 20 minutes to complete.

  • Medical Chart Review
    Instrument used for follow-up.  Basically the same measure described in the baseline data collection with very little modification.

  • Medical Chart Review Instructions
    Provides detailed directions on how to use the medical chart review instrument.

  • Written Clinician Survey
    A questionnaire used to capture information from health care providers about the care and treatment of patients with diabetes that includes questions on access to disease management services, service restrictions, physician compensation, self-reported decision-making style, and demographic characteristics. The survey consists of 15 questions, including skip patterns and takes approximately 10 minutes to complete.
